We are seeking an experienced and motivated Registered Children's Nurse to join our Community Children's Nursing Team as a Band 6 Community Children's Nurse on a Fixed Term contract. The successful candidate will be responsible for providing high-quality nursing care to children with short and long-term conditions within their own homes and wider community.
Main Responsibilities- Lead and manage a caseload of children with complex health needs, providing individualized care and support to ensure optimal health outcomes.

- Develop and implement individualized care plans, taking into account the child's unique needs, preferences, and circumstances.
- Provide education and support to children, parents, and carers on various health conditions, promoting self-management and empowerment.
- Conduct regular assessments and reviews to monitor the child's progress, adjusting care plans as needed to ensure optimal outcomes.
- Work collaboratively with other healthcare professionals to ensure continuity of care and effective communication.
- Participate in quality improvement initiatives, contributing to the development of best practices and policies that enhance the quality of care.

Requirements- Registered Children's Nurse with a valid NMC registration.
