Job Overview

Berkshire Healthcare NHS Foundation Trust is seeking a motivated Mental Health Practitioner to join our friendly and innovative team in the Windsor, Ascot and Maidenhead (WAM) Older Peoples Mental Health Service.

Main Duties
  • Participate as a member of a multi-disciplinary team managing a caseload of patients.
  • Undertake assessments of patients, involving their carers and family members in the patient's care and care plans.
  • Formulate, execute, and evaluate care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
  • Supervise Support Workers and/or students.
  • Provide advice to other members of the service and agencies on the needs of service users.
