Join Our Team as a Care Coordinator

We are seeking an experienced Care Coordinator to join our dynamic team at Active Care Group Recruitment. As a Care Coordinator, you will be responsible for delivering high-quality care services to our clients in their own homes.

Key Responsibilities:
  • Develop and implement care plans that meet the unique needs of each client
  • Work closely with our clinical and business leadership teams to ensure seamless care delivery
  • Build strong relationships with clients, families, and external stakeholders
  • Ensure accurate and timely record-keeping and reporting
  • Support the recruitment and training of support workers
Requirements:
  • NVQ 4 or above in Health and Social Care
  • Full clean driving license with access to own car
  • Excellent communication and interpersonal skills
  • Experience in care at Team Leader level or above
  • Previous management experience would be an advantage
