Key Responsibilities
The ideal candidate will possess a degree in Advanced Clinical Practice and relevant clinical expertise to operate independently, ensuring ongoing patient care and effectively managing acute healthcare demands. You should have substantial experience and be a certified independent prescriber, enabling you to autonomously assess, diagnose, treat, and refer patients as necessary. You will be responsible for managing your own patient appointment schedule daily, alongside undertaking specific duties in various practice areas.
About Our Practice
Our healthcare facility:
- Serves a patient population of 13,078
- Comprises a team of 4 GP partners, 5 salaried GPs, and 2 GP trainees, supported by a management team, reception staff, clinical pharmacist, paramedic, social prescribers, and a nursing team.
- Utilizes EMIS web for patient management.
We have recently adopted a comprehensive triage model:
- This model ensures that every patient contacting the practice is initially assessed before scheduling an appointment with a GP partner.
- The goal of this approach is to address patient needs with the most suitable professional or service while minimizing unnecessary transitions.
- This triage system aims to help practices meet patient demand within their current capacity by leveraging the diverse skills of the broader team, utilizing online services, telephone, and video consultations, as well as in-person visits when necessary.
- It recognizes that a significant portion of healthcare needs presented to general practice can be effectively managed by other professionals or services, or even by the patients themselves with the right information.
- The three essential components of our triage model include: understanding patient needs, administrative signposting, and clinical assessment.
Job Duties
You will be responsible for managing your daily appointment schedule, prescribing medications, and making necessary referrals or follow-up actions as required.
Assess and treat patients seeking healthcare services, making appropriate referrals to other members of the primary and secondary healthcare teams. Ensure that clinical practices are safe, effective, and within your scope of competence, while recognizing your limitations. Provide guidance to patients regarding general health and ongoing medical needs, referring them to other healthcare professionals as necessary. Participate in the administrative and professional responsibilities of the healthcare team. Maintain accurate and clear records of all consultations and treatments in patient files at the time of service. Attend and engage in practice meetings as required. Be prepared to work flexible hours when necessary to accommodate staff absences and special clinics.
Qualifications
Essential
- Recognized qualification in Advanced Clinical Practice (degree/diploma)
- Registered General Nurse (currently registered with the Nursing & Midwifery Council)
Desirable
- Experience as an Advanced Clinical Practitioner (1 year preferred)
