Palliative Care Coordinator

We are seeking a highly motivated individual to join our team as a Specialist Care Co-ordinator. This is an exciting opportunity to work with the Specialist Palliative Care Team (SPCT) and End of Life Care Team. The successful candidate will strive to provide care that is both of a high standard and in line with National and local guidelines.
Main ResponsibilitiesThe post holder will be responsible for providing secretarial and administrative support to the SPCT. This will include coordinating fast track care plans, working in collaboration with local Integrated Care Boards (ICBs) to commission care packages for home, care homes, and Saint Francis Hospice to meet patients' needs.
Key Skills and Qualities- Excellent organisational skills
- Ability to communicate with the public and health professionals with understanding and empathy
- Knowledge of Careflow
- Ability to work independently and as part of a team
- Experience of working in a healthcare setting
- Ability to work in a fast-paced environment
- Good computer literacy
