About Saint Francis Hospice

We are a leading charity providing specialist palliative care services to patients with life-limiting illnesses.

Job Summary:

  • Provide expert advice and support to patients and healthcare professionals in a multidisciplinary environment.
  • Deliver care via telephone, home visits, or nurse-led clinics, operating in a 24/7 service.
  • Collaborate with primary care teams to provide specialist support and conduct telephone assessments.
  • Work autonomously and as part of a team to deliver palliative care services, utilizing non-medical prescribing qualifications.

Main Responsibilities:

  • Respond to referrals and provide holistic support to patients with life-limiting illnesses.
  • Offer specialist advice to healthcare professionals and patients, prioritizing workload efficiently.
  • Manage caseloads and participate in education and quality initiatives to improve palliative care services.
  • Lead audits and contribute to service development, ensuring high-quality care for patients.

Requirements:

  • Excellent knowledge of symptom control and experience in palliative care.
  • Ability to work autonomously and as part of a team, with strong communication and collaboration skills.
  • Post-registration experience as a RGN, non-medical prescribing qualification, and a degree in a related field.
  • Teaching qualification or equivalent experience, with a passion for delivering exceptional care to those in need.
