Primary Care Physician

The Elms Surgery is seeking a highly skilled and dedicated Salaried General Practitioner to join our team. As a key member of our primary care team, you will be responsible for providing high-quality patient care and contributing to the overall success of our practice.
Key Responsibilities- Assessment and Diagnosis: Conduct thorough assessments of patients with undifferentiated and undiagnosed problems, utilizing your clinical expertise to develop effective care plans.
- Long-term Condition Management: Develop and implement care plans for patients with chronic conditions, ensuring seamless coordination of care and optimal patient outcomes.
- Counselling and Education: Provide empathetic and informative counselling to patients, empowering them to make informed decisions about their health and well-being.
- Caseload Management: Admit and discharge patients from your caseload, referring to other care providers as necessary to ensure comprehensive care.
- Prescribing and Medication Management: Prescribe medications in accordance with our practice formulary and ICB guidelines, ensuring safe and effective use of medications.
- Supervision and Support: Provide guidance and support to Physician Associates, ensuring the smooth operation of our practice and optimal patient care.
- Palliative and Elderly Care: Manage and support ongoing palliative and elderly care case loads, prioritizing patient comfort and dignity.
