Job Summary
We are seeking an experienced Advanced Nurse Practitioner to join our team on a full-time, permanent basis. The successful candidate will be responsible for developing and implementing health promotion and wellbeing programs, managing patients with acute and chronic medical conditions, and providing advanced nursing care.
Main Responsibilities
- Develop and implement health promotion and wellbeing programs to improve patient outcomes.
- Manage patients with a range of acute and chronic medical conditions, providing subject matter expert advice.
- Implement and evaluate individualized treatment plans for chronic disease patients.
- Identify and manage patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Provide advanced, specialist nursing care to patients as required, in accordance with clinical evidence, NICE, and the NSF.
- Provide wound care, including ulcer and Doppler services.
- Provide specialist clinics, such as ear syringing and contraception services.
- Collect pathological specimens and process and interpret pathology and other test results as required.
- Provide chronic disease clinics, delivering patient care as necessary, and referring patients to secondary or specialist care as required.

Person Specification
We are looking for an experienced and qualified Advanced Nurse Practitioner with the following skills and qualifications:
- Registered Nurse with the Nursing and Midwifery Council.
- Qualified Independent Nurse Prescriber on the NMC register.
- Masters degree required for qualification post December 2020, refer to RCN Credentialing for Advanced Level of Nursing Practice.
- Minor illness qualification.
- Experience of working as a practice nurse or community nurse.
- Wound care and removal of sutures and staples.
