Job Summary:

We are seeking an experienced Advanced Nurse Practitioner to join our Primary Care Network team in North Watford. As a key member of our clinical team, you will be responsible for providing high-quality patient care and contributing to the development of our health promotion and wellbeing programmes.

Main Responsibilities:

  • Develop and implement health promotion and wellbeing programmes for our patients.
  • Manage patients with acute and chronic medical conditions, providing expert advice and care.
  • Implement and evaluate individualised treatment plans for patients with chronic diseases.
  • Identify and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
  • Provide advanced nursing care to patients, including wound care and specialist clinics.
  • Undertake the collection of pathological specimens and process and interpret test results.
  • Provide travel medicine services and request pathology services as necessary.

Person Specification:

Essential Qualifications:

  • Registered Nurse with the Nursing and Midwifery Council.
  • Qualified Independent Nurse Prescriber on the NMC register.
  • Masters degree required for qualification post December 2020.
  • Minor illness qualification.

Essential Experience:

  • Experience of working as a practice nurse or community nurse.
  • Wound care and removal of sutures and staples.
  • Experience of working with patients with acute and chronic medical conditions.
