Main Responsibilities- Provide comprehensive medication support to practices, addressing patient queries and concerns.
- Contribute to the repeat prescription process, ensuring effective management of patient medication.
- Support chronic disease management, medication reviews, and polypharmacy management.
- Collaborate with healthcare professionals to manage patients with long-term conditions.
- Lead medication reviews and safe prescribing practices in care homes.
- Offer patient-facing and telephone support for medication-related queries.
- Implement changes in response to MHRA alerts and national guidance.
- Provide advice on shared care, NICE guidance, and RAG lists.
- Manage common and minor ailments within scope.
- Oversee discharge summaries from hospitals and community services.
Our Primary Care Network is a forward-thinking organization, delivering care to over 25,000 registered patients. As a Clinical Pharmacist, you will become an integral part of our diverse clinical team, working in a dynamic and innovative environment.
Person Specification- Essential:
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
- Minimum of 2 years experience as a pharmacist, demonstrated with a practice portfolio.
- Desirable:
- Experience and awareness of common acute and long-term conditions.
- Independent prescribing qualification (or working towards).
- Ability to follow organizational policies and codes of conduct.
