Job Title: Adult Mental Health Practitioner

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our team at NELFT North East London Foundation Trust. As a key member of our Integrated Crisis Assessment Hub, you will play a vital role in providing high-quality care to individuals experiencing mental health crises.

Key Responsibilities:
  • Assess and support individuals presenting to the Hub, ensuring their physical and mental health needs are met.
  • Work closely with the ICAH Health Care Support Worker to monitor clients' physical health state and maintain a safe environment.
  • Complete the triage tool and assist the ICAH Clinical lead in gathering service user feedback.
  • Contribute to maintaining accurate record-keeping and outcomes through the Hub Admission logbook.
  • Assist with telephone triaging, taking referrals, and addressing patient care queries.
Requirements:
  • Registered Mental Health Nurse with a minimum of 6 months experience in an inpatient setting.
  • NVQ Level 2 or equivalent standard of literacy and numeracy.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a multi-disciplinary team.
  • A clear understanding of the Crisis Resolution Model and related legislation, codes of practice, and guidance.
  • Ability to work independently and as part of a team, with a strong commitment to equal opportunities in service delivery.
