About the Role

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our Integrated Crisis Assessment Hub team. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to individuals experiencing mental health crises.

Key Responsibilities
  • Assess and support individuals presenting to the Hub, ensuring their physical and emotional needs are met.
  • Work closely with the ICAH Health Care Support Worker to monitor clients' physical health and safety.
  • Complete the triage tool and ensure accurate record-keeping of outcomes.
  • Assist the ICAH Clinical Lead in completing service user feedback and maintaining the Hub Admission logbook.
  • Provide telephone triage support, taking referrals and addressing patient care queries.
Requirements
  • Registered Mental Health Nurse with a minimum of 6 months' experience in an inpatient setting.
  • NVQ level 2 or equivalent standard of literacy and numeracy.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • A clear understanding of the Crisis Resolution Model and related legislation.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
