Job Summary:
We are seeking a highly skilled and compassionate Senior Social Worker to join our Adult Social Care Service in Lancashire County Council. As a Senior Social Worker, you will be responsible for providing holistic, face-to-face Care Act Assessments, reviews and support planning, using a strengths-based approach to ensure that people receive the right support at the right time.
Key Responsibilities:
- Conduct comprehensive assessments of individuals' needs, identifying areas of strength and vulnerability, and develop personalized support plans to promote independence and well-being.
- Work collaboratively with a multidisciplinary team to prevent hospital admissions and support people to remain at home, utilizing the Discharge to Assess model to ensure that assessments are done away from the hospital environment.
- Develop and maintain effective relationships with service users, their families, and other stakeholders, ensuring that their needs are met and their voices are heard.
- Stay up-to-date with relevant legislation, policies, and procedures, applying this knowledge to ensure that assessments and support plans are compliant and effective.
Requirements:
- Proven experience of working in adult social care, with a strong understanding of the Care Act and its implications.
- Excellent assessment and planning skills, with the ability to work collaboratively with others to achieve positive outcomes.
- Strong communication and interpersonal skills, with the ability to build effective relationships with service users, families, and other stakeholders.
- Ability to work in a fast-paced environment, prioritizing tasks and managing competing demands to meet deadlines.
