Role Responsibilities

In this role, you will engage with clients through personalized support, comprehensive care planning, risk evaluation, and collaborative efforts with various agencies.

Your responsibilities will also include facilitating group sessions and coordinating with our partners to enhance each client's overall health and wellness.
