Adult Social Care Specialists

Job Summary
Cheshire East Council is seeking self-motivated Social Care Assessors to join our innovative and friendly Adult Social Care Teams. These are temporary posts initially for a period of 12 months and will be reviewed as required.
About the Role
We are looking for enthusiastic Social Care Assessors who have an understanding of adult social care, mental capacity, and safeguarding issues. The ideal candidate will be self-motivated with excellent communication skills and the ability to develop and work both autonomously within Adult Community Services and the wider multi-disciplinary arena.
Key Responsibilities
- Conduct thorough assessments of individuals to identify their needs and develop personalized support plans.
- Work collaboratively with other professionals, including healthcare providers, social workers, and community organizations, to ensure seamless service delivery.
- Provide guidance and support to individuals, families, and carers to empower them to make informed decisions about their care and well-being.
- Maintain accurate and comprehensive client documentation, adhering to Council policies and professional practice.
- Participate in training and professional development opportunities to enhance skills and knowledge in adult social care.
Requirements
- Relevant qualifications in health and social care, such as a degree or diploma in social work, nursing, or a related field.
- Experience working in adult social care, preferably in a community setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
- Ability to work autonomously and as part of a team, with a flexible approach to work arrangements.
- Valid driving license and access to a vehicle, as regular travel is required.
