We are seeking an experienced Registered Nurse to join our Reactive Emergency Assessment Community Team (REACT) as a Senior Registered Nurse and lead our crisis team. Main Responsibilities- Conduct Face-to-Face Patient Assessments
Conduct thorough face-to-face patient assessments, deliver planned treatment, and monitor medical conditions for patients with acute deterioration, exacerbations of long-term conditions, palliative/end-of-life care, and rehabilitation needs.
Triaging and PrioritizationBe a key player in REACT, triaging, and prioritizing patients with good clinical and diagnostic skills, including venepuncture, cannulation, ECGs, catheter care, end-of-life care, and line care.
Collaboration with Multidisciplinary TeamWork with the wider multidisciplinary integrated team in the community to prevent unnecessary hospital admissions.
Excellent Communication SkillsDevelop excellent communication skills to ensure the best possible experience and outcomes for crisis patients, including conducting excellent handovers, escalating, and presenting patient cases within the VW MDT.
Collaboration with Healthcare ProfessionalsCollaborate with all healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.

Person Specification- Essential:
- Experience leading teams.
- Clinical experience in admission avoidance/treating acutely ill patients in their own home.
- Coaching/Mentoring.
- Managing change.
- Evidence of managing and improving the patient experience.
- Desirable:
- Previous experience at Band 6.
- Evidence of involvement in research and audit.
- Evidence of teaching.
- Interviewing/recruitment skills.
- Qualifications:
- Registered Nurse Qualification.
- Evidence of post-registration training.
- ILS certificate as relevant to the area.
- Student assessor.
- Desirable:
- Leadership qualification.

- Willingness to undertake further academic study.
- Knowledge:
- Clinical expertise (cannulation, PICC line care, catheter care, wound care, diabetes management, end-of-life care, etc).
- Evidence of professional competencies in a multidisciplinary setting.
- Evidence of management of complex patients.
- Ability to travel in the community (pool cars available).
