Palliative Care Specialist - Triage and Referral Management

We are seeking an experienced and skilled Palliative Care Specialist to join our team as a Triage and Referral Manager. As a key member of our First Contact team, you will play a vital role in receiving and managing referrals made to our hospice, ensuring timely and effective care for patients and their families.

Key Responsibilities:
  • Receive and triage incoming referrals, assessing patient needs and determining the appropriate level of care required.
  • Utilize agreed resources and guidelines to effectively assess, prioritize, advise, schedule patient care management, or refer patients when necessary to other appropriate health and social care agencies.
  • Manage all aspects of the referral system to the hospice, identifying appropriateness, urgency, and level of priority.
  • Work collaboratively with internal and external partners to support the coordination and management of patients referred to the hospice, ensuring care is planned to meet individualized and dynamic needs.
  • Use assessments and interventions as opportunities to train, coach, and mentor clinical staff and visitors to the hospice about specialist palliative care practice.
Requirements:
  • Registered Nurse with bachelor's degree level training in palliative/end of life care or related subject and extensive experience working within a specialist palliative care field.
  • Advanced assessment skills.
  • Critical Thinking Skills.
  • Able to demonstrate knowledge of disease trajectories and treatment effects.
  • Advanced Communication skills.
  • Enthusiasm and a flexible attitude to working.
  • Ability to work autonomously and as part of a team.
