Clinical Specialist Community Learning Disability Nurse

We are seeking a highly skilled Clinical Specialist Community Learning Disability Nurse to join our team at Sirona care & health CIC. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality support to individuals with learning disabilities and their carers.
Key Responsibilities- Assess and manage the health needs of adults with learning disabilities
- Provide specialist advice and support to other staff and teams
- Develop and maintain expertise in specific clinical areas
- Coordinate the multi-professional team and work closely with the Clinical and Operational Lead
- Identify and plan training for other professionals and carers
We are looking for an individual with:
- Highly competent clinical reasoning skills and effective communication
- Ability to work collaboratively within multidisciplinary teams
- Clear and competent prioritisation skills and ability to manage caseload
- Significant experience in community settings and learning disability nursing
- Current NMC registration and documented evidence of CPD
