Job Summary

We are seeking a highly skilled and experienced Community Mental Health Nurse to join our team at Hertfordshire Partnership University NHS Foundation Trust. As a Community Mental Health Nurse, you will be responsible for delivering high-quality care to individuals with mental health needs, working closely with other healthcare professionals to provide comprehensive assessments, care planning, and interventions.

Main Responsibilities
  • Conduct comprehensive assessments of individuals with mental health needs, identifying their strengths and challenges, and developing personalized care plans.
  • Deliver evidence-based interventions, including medication management, psychotherapy, and social support, to promote recovery and well-being.
  • Collaborate with other healthcare professionals, including psychiatrists, psychologists, and social workers, to provide comprehensive care and support.
  • Develop and maintain effective relationships with service users, their families, and carers, ensuring that their needs are met and their voices are heard.
  • Contribute to the development and implementation of care pathways, policies, and procedures, ensuring that they are evidence-based and effective.
  • Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.

Person Specification
  • Registered Mental Health Nurse with a valid NMC registration.
  • Experience of working in a community mental health setting, with a strong understanding of mental health needs and the ability to provide high-quality care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and carers.
  • Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality care and support.
  • Knowledge of mental health legislation, policies, and procedures, with the ability to apply this knowledge in practice.
