We are seeking an experienced and skilled Community Mental Health Nurse to join our team at Hertfordshire Partnership University NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality care to individuals with mental health needs in the community.
Key Responsibilities- Provide comprehensive assessments and care planning for individuals with mental health needs
- Develop and implement individualized care plans in collaboration with service users, families, and other healthcare professionals
- Deliver evidence-based interventions and treatments to promote recovery and well-being
- Work collaboratively with other healthcare professionals to ensure seamless care coordination and transitions
- Provide education and support to service users, families, and carers to promote mental health and well-being
- Contribute to the development and implementation of service policies and procedures
- Participate in quality improvement initiatives and contribute to the evaluation of service effectiveness
- Registered Mental Health Nurse (RMN) or equivalent qualification
- At least 2 years of post-registration experience in a mental health setting
- Experience of working in a community mental health team
- Knowledge of mental health legislation and policies
- Ability to work autonomously and as part of a multidisciplinary team
- Excellent communication and interpersonal skills
