Key Responsibilities:

As a Young Persons and Adult Link Worker, you will be providing social prescribing support to young people, and/or their families, and a cohort of adults. The aim of the role is to help clients improve their health and wellbeing by connecting them to community support. This could include helping people with a range of issues such as coping with social pressures, emotional wellbeing, physical inactivity, financial difficulties, crime, loneliness, bullying, relationship issues, housing issues, and family problems.

  • Knowledge of local community resources and services
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
