Main Responsibilities- Conduct comprehensive assessments of patients' needs, developing and implementing personalized care plans to promote independence and well-being.
- Deliver high-quality nursing care, including administering medications, wound care, and end-of-life care, in accordance with Trust policies and procedures.
- Work collaboratively with patients, families, and healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Participate in clinical audits and quality improvement initiatives to enhance patient safety and care.
- Contribute to the development and implementation of new care pathways and protocols to improve patient outcomes and experience.
- Registered Nursing Associate on the NMC register.
- Recent experience in community nursing or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to prioritize tasks, manage time, and maintain accurate records.
