Job Summary
The Park Avenue Medical Centre is seeking an experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. As a key member of our primary care team, you will provide high-quality patient care, working collaboratively with our healthcare professionals to deliver exceptional patient outcomes.
Key Responsibilities
Clinical Practice
- Conduct comprehensive clinical assessments and develop effective treatment plans for patients with acute and chronic conditions.
- Prescribe and review medication, ensuring safe and effective treatment.
- Work with patients to promote healthy lifestyles and self-care.
- Provide sensitive communication and support to patients with complex needs.
- Collaborate with the healthcare team to ensure seamless care delivery.
Communication
- Develop and maintain effective relationships with patients, carers, and healthcare professionals.
- Communicate complex information in a clear and concise manner.
- Anticipate and address communication barriers to ensure patient understanding.
Delivering a Quality Service
- Work within your scope of practice, adhering to professional codes and standards.
- Contribute to quality governance, participating in audits and evaluations.
- Maintain accurate and contemporaneous patient records.

- Act as a clinical leader, supporting the duty team and Practice Management.
- Problem-solve clinical matters, communicating with Directors as needed.
- Take the lead in documenting, investigating, and reporting significant events.

Person Specification
Essential
- Registration with a professional body (NMC/HCPC).
- Evidence of education at level 7 or Digital Badge.
- Non-Medical Prescriber.
- Experience working clinically as an Advanced Practitioner in Primary or Emergency care.
