Job Summary
The Park Avenue Medical Centre is seeking an experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. As a key member of our primary care team, you will provide high-quality patient care, working collaboratively with our healthcare professionals to deliver exceptional patient outcomes.
Key Responsibilities
Clinical Practice
- Conduct comprehensive clinical assessments and develop effective treatment plans for patients with acute and chronic conditions.
- Prescribe and review medication, ensuring safe and effective use.
- Work with patients to promote healthy lifestyles and self-care.
- Provide education and support to patients and carers on medication management and health promotion.
- Identify and manage patients at risk of developing long-term conditions.
Communication
- Develop and maintain effective communication skills to ensure patients are fully informed and engaged in their care.
- Communicate with patients and carers who require sensitive or complex information.
- Work collaboratively with the healthcare team to ensure seamless communication and care coordination.
Leadership and Personal Development
- Act as a clinical leader, providing daily support and guidance to the nursing team.
- Develop and implement quality improvement initiatives to enhance patient care and outcomes.
- Participate in clinical supervision and mentorship to support the development of junior staff.
Team Working
- Work collaboratively with the healthcare team to achieve shared goals and objectives.
- Develop and maintain effective relationships with patients, carers, and colleagues.
- Contribute to the development of a positive and supportive team culture.

Person Specification
Essential
- Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC).
- Education at level 7 or higher.
- Experience of working as an Advanced Practitioner in primary or emergency care.

- Experience of managing long-term conditions.

- Research experience.
