Position Overview

Key Responsibilities

This NHS initiative is designed to assist patients in transitioning from hospital care to home once they are medically stable and can be safely managed within the community. Throughout this process, patients will continue to receive care from their consultant teams.

As a crucial member of our team, your responsibilities will include providing direct patient care within their homes. This role will primarily focus on the new Blood Transfusion pathway, necessitating an adaptable and innovative individual who is comfortable working in a dynamic environment.

Qualifications and Experience

Essential:

  • Current NMC Registration
  • BSc or equivalent experience
  • Understanding of EIM
  • Excellent communication abilities
  • Nurse Prescriber (or willingness to pursue)
  • Strong clinical and holistic assessment skills
  • Awareness of current NHS issues
  • Knowledge of infection prevention and control
  • Experience in Community/A&E settings
  • Diverse clinical experience portfolio

Desirable:

  • Familiarity with transfusion practices
  • Experience in audit/change initiatives
