About the Role

We are seeking a highly skilled and dedicated Mental Health Nurse to join our team as a B6 Memory Assessment Nurse in our Memory Assessment Service (MAS).

This is a unique opportunity to work with individuals experiencing cognitive changes and concerns regarding dementia. As a key member of our multidisciplinary team, you will be responsible for undertaking comprehensive assessments, delivering diagnoses, and formulating treatment plans.

You will work in a clinic and community care setting, dividing your time equally between these two areas due to the rural geographical location. This role requires the ability to work autonomously, conduct patient home visits, and make complex decisions unsupervised.

Key Responsibilities
  • Undertake comprehensive assessments of patients with possible organic mental health problems
  • Deliver diagnoses and formulate treatment plans
  • Work collaboratively with the multidisciplinary team to provide holistic care
  • Conduct patient home visits and work in a community care setting
  • Make complex decisions unsupervised and work autonomously
