Position Overview

We are seeking a Band 7 Lead Physiotherapist to oversee the Community Rehabilitation Services within our North Integrated Community Rehabilitation Team.

In collaboration with the Band 7 Occupational Therapy Lead, your primary responsibilities will encompass professional guidance and operational oversight of the teams, ensuring the assessment, planning, and implementation of evidence-based rehabilitation and reablement strategies. You will work closely with your Primary Care Network (PCN) counterparts in Nursing and Adult Social Care to enhance an effective, integrated multi-disciplinary approach within the local teams.

If you are a qualified Physiotherapist with substantial experience in healthcare settings, this role presents an excellent opportunity to leverage your leadership and management skills to promote rehabilitation excellence within our community.

Key Responsibilities

  • Lead and manage the operational functions of the Community and Inpatient Therapy teams in East Cornwall, aligning with local health needs and the strategic priority of delivering services closer to home, particularly focusing on the Inpatient ward.
  • Ensure the provision and maintenance of high-quality, evidence-based care that meets the needs and preferences of patients, caregivers, and families.
  • Develop and maintain positive relationships with local commissioning bodies and community organizations.
  • Create and sustain an effective, integrated multi-disciplinary approach within the locality teams.
  • Ensure adherence to best practice guidelines and performance standards while achieving service targets.
  • Function as an autonomous practitioner responsible for the assessment and treatment of patients with complex health and social care needs.
  • Provide developmental leadership and management to all team members within the locality, contributing to future service development.
  • Support the Integrated Care Team Manager (ICM) by accepting delegated responsibilities for specific tasks.

Qualifications and Skills

Essential:

  • Current professional registration with HCPC.
  • Professional qualification as a Physiotherapist.
