Position Overview
We are seeking dedicated individuals who possess a strong commitment to customer service and a genuine enthusiasm for delivering exceptional patient support. As an integral member of our Integrated Urgent Care Service team, which operates around the clock, your responsibilities will include the coordination of patient care services. This encompasses the management of our Treatment Centres, the dispatching of ambulances and response vehicles, and the oversight of various other duties through effective communication.

Work Schedule
Positions are available ranging from 16 hours per week to full-time, with shift patterns that will include a mix of overnight, weekend, and holiday shifts, accompanied by pay enhancements for unsociable hours.
A minimum commitment of three weekends within a four-week period is expected, with opportunities for overtime also available.
Key Responsibilities
The role of Patient Care Coordinator is essential to the execution of our NHS contracts, including Acute GP, NHS 111, and ambulance transport services. In this exciting position, you will:
- Organize treatment centres and manage the dispatch of ambulances and response vehicles.
- Coordinate patient care services in collaboration with healthcare professionals.
- Handle calls efficiently, utilizing our computer system to ensure timely resolution of inquiries.
- Maintain accurate records of patient information and symptoms.
- Prioritize and schedule home visits and treatment centre appointments based on clinical needs.
- Monitor demand for home visits and appointments to ensure safety and compliance with performance targets.
- Identify patients eligible for care navigation to appropriate services.
- Provide professional navigation to patients, guiding them to suitable healthcare services.
- Develop a comprehensive understanding of local healthcare services and resources.
- Manage challenging interactions with professionalism and effective communication.
- Collaborate with the Service Delivery Manager to address clinical or operational issues.
- Ensure compliance with all statutory and mandatory training requirements.
