As a Community Care Support Specialist, you will play a crucial role in our innovative approach to care. Your Responsibilities Will Include:
  • Adapting to flexible working hours and responding to dynamic changes.
  • Providing essential practical and emotional support to individuals in their homes, assessing their specific needs.
  • Collaborating with clients, their families, and other professionals to develop personalized care plans.
  • Assisting individuals with daily living activities, including personal hygiene, meal preparation, medication management, and mobility support.
  • Working alongside healthcare professionals and social workers to ensure a cohesive approach to care.
  • Monitoring and evaluating clients' progress, making necessary adjustments to care plans to support optimal recovery.
  • Maintaining comprehensive documentation of interactions, care plans, and client progress.

To Succeed in This Role, You Will Need:
  • Experience in a similar care role, ideally within a healthcare or social care environment.
  • A compassionate and empathetic demeanor, with a commitment to positively impacting individuals' lives.
  • Strong communication and interpersonal skills, enabling effective teamwork.
  • Understanding of person-centered care principles and a dedication to preserving dignity and autonomy.
  • Ability to work independently, demonstrating initiative and reliability.
  • Flexibility to work various shifts, including evenings and weekends.
  • A full driving license and access to a vehicle is preferred but not mandatory.
