Transform Community Development is seeking a highly motivated and dedicated Support Worker to join our dynamic team within our supported temporary accommodation. As a valued member of our team, you will play a crucial role in delivering a high-quality programme of support that promotes participant choice, control, and independence.
Main Responsibilities- Liaise with local authorities and other stakeholders to facilitate referrals and ensure seamless service delivery.
- Conduct thorough assessments to identify participant support needs and develop comprehensive support plans in collaboration with participants.
- Implement and review support plans, ensuring timely and effective interventions to address participant needs.
- Establish and maintain strong relationships with participants, colleagues, and partners to ensure a cohesive and supportive environment.
- Collaborate with other agencies to provide holistic support and connect participants with relevant resources.
- Attend Multi-Disciplinary Team meetings to share knowledge and best practices.
- Support participants with daily tasks, appointments, and medication management.
- Advocate for participants' rights and interests when necessary.
- Complete all administrative tasks, including but not limited to:
- Housing Support Assessment Tool
- Support plan and risk assessments
- Housing benefit and other benefits paperwork
- Reporting statistics to local authorities and internal stakeholders
- Referral paperwork to partnership agencies
- Support with correspondence for participants
- Handling and recording of monies for safekeeping and/or service charges
