Position: Care Support Specialist
Service: Housing & Community Support Services

Location: Based at The Action Group, with flexibility for regional assignments as determined by management.

Reporting to: Team Manager

Compensation:

Unqualified: £24,402.92 - £24,890.98 per annum pro rata
Qualified: £24,890.98 - £25,338.37 per annum pro rata
Casual Rate: £12 per hour
Sleepover Rate: £96 per night (if applicable)

Hours: Up to 16 hours per week

Role Purpose:

The primary objective of this role is to deliver compassionate, efficient, and effective support services to individuals utilizing The Action Group's offerings. This includes aiding individuals in skill development through active participation in various support activities, thereby enriching their experiences and promoting autonomy.

Specific Duties

  • Implement the TAG Key Worker system, coordinating support plans and case reviews with all relevant stakeholders.
  • Enhance the physical and emotional wellbeing of those we support by promoting self-care and conducting risk assessments.
  • Build meaningful and trusting relationships with individuals, helping them gain insights into their behaviors and reactions.
  • Encourage independence in daily living activities, providing support only when necessary.
  • Promote hobbies and community involvement, including organizing outings and accompanying individuals to activities.
  • Assist with personal care as required.
  • Facilitate effective communication and self-advocacy for the individuals we support.
  • Advocate for individuals when necessary, liaising with external organizations regarding benefits, education, and employment.
  • Engage with relatives and caregivers, ensuring their involvement in the support process.
  • Research local services and resources available to those we support.
  • Participate in flexible shift patterns to meet the needs of individuals, including evenings and weekends.
  • Occasionally cover for absent Support Workers in other teams.

General Duties

  • Adhere to TAG's Mission, Aims & Objectives, and current Business Plan.
  • Manage work time effectively and maintain organized filing systems.
  • Communicate promptly and clearly with colleagues and individuals.
  • Stay updated and comply with TAG policies and procedures.
  • Promote individual rights and responsibilities at all times.
  • Maintain confidentiality as required.
  • Assist colleagues and contribute to a positive work environment.
  • Engage in Continuous Professional Development (CPD) and support TAG in maintaining the Investors In People standard.
  • Prioritize work-life balance.
