Role: Care Support Specialist
Service: Housing & Community Support Services
Base: Regional base as determined by management
Reporting to: Team Manager
Compensation:
Unqualified: £24,402.92 - £24,890.98 per annum pro rata
Qualified: £24,890.98 - £25,338.37 per annum pro rata
Casual Rate: £12 per hour, with additional rates for overnight duties.
Hours: Up to 16 hours per week
Key Responsibilities:
- Implement the Key Worker system, coordinating support plans and ensuring effective communication among all stakeholders.
- Support the physical and emotional wellbeing of individuals, conducting assessments and promoting self-care.
- Build meaningful relationships with those we support, fostering trust and understanding.
- Encourage independence in daily activities, providing assistance only when necessary.
- Facilitate participation in hobbies and community life, including organizing outings and activities.
- Assist with personal care as required.
- Enhance communication skills and advocate for individuals' needs with external organizations.
- Engage with families and caregivers to ensure their involvement in the support process.
- Research local services and resources available to individuals.
- Adapt to various shift patterns to meet the needs of those we support.
- Occasionally cover for other team members as needed.

- Maintain organized records and comply with all relevant policies.
- Communicate clearly and respectfully with all stakeholders.
