Job Title: GP Pharmacist - Clinical Leadership

We are seeking an experienced GP Pharmacist to join our team as a Clinical Leader. The successful candidate will have a strong background in clinical pharmacy and a passion for leading and developing multidisciplinary teams.

About the Role

The GP Pharmacist will work closely with our team of pharmacists and healthcare professionals to provide clinical leadership and expertise in medication management, quality improvement, and patient care. The role will involve working with patients, practice staff, and other healthcare professionals to optimize medication use and improve patient outcomes.

Key Responsibilities
  • Provide clinical leadership and expertise in medication management, quality improvement, and patient care
  • Work closely with the multidisciplinary team to develop and implement medication management plans
  • Conduct structured medication reviews and provide expert advice on medication use
  • Develop and implement quality improvement initiatives to improve patient outcomes
  • Collaborate with practice staff and other healthcare professionals to optimize medication use and improve patient care
Requirements
  • Minimum of 5 years post-registration experience with at least 2 years in General Practice
  • Certificate in Non-Medical Prescribing
  • Experience of using prescribing qualification in General Practice
  • Evidence of further education in clinical pharmacy, e.g. Completion of NHS Pathway Training for Practice Pharmacists or a Clinical Pharmacy Diploma
