About the Role
The successful candidate will be an integral part of a compassionate care team, delivering community-based palliative nursing services. This role involves working closely with families of children and young individuals facing life-threatening and life-limiting conditions.
The post holder will provide comprehensive, holistic nursing care tailored to the needs of children and young people with complex medical requirements, whether in their homes or other respite care settings, including hospital environments. Emotional support and guidance will be offered to both the children and their families, ensuring a nurturing approach to care.
Key Responsibilities
- Deliver exceptional, individualized holistic care to children and young people with life-limiting conditions and their families, fostering empowerment and collaboration.
- Supervise and support the child or young person in the absence of their parents or guardians.
- Maintain effective communication with all members of the care team.
- Promote evidence-based practices that enhance the quality of care provided to children on the caseload, adhering to clinical governance standards.
- Perform all necessary nursing duties within the home setting, while developing and delivering additional clinical skills as required.
- Manage your own workload efficiently during respite visits and office responsibilities.
- Adhere to UHBW policies and Jessie May guidelines in all aspects of care.
- Facilitate effective collaboration with colleagues and partner organizations.
- Work alongside your line manager to enhance service delivery and improve care quality for children and young people.
- Ensure compliance with Health and Safety regulations, reporting any incidents to your line manager.
