Job Title: Advanced Clinical Practitioner

Service Care Solutions - Healthcare is seeking an experienced Advanced Clinical Practitioner to join our team in an Urgent Care Centre in Walsall.

About the Role:

We are looking for a highly skilled and motivated Advanced Clinical Practitioner to work within our Urgent Care Centre. The successful candidate will be responsible for delivering evidence-based nursing services, working as part of a multi-disciplinary team, and providing care to patients within their scope of practice.

Key Responsibilities:
  • Deliver health promotion and wellbeing programmes to patients
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions
  • Support the clinical team in the development of individual treatment plans for chronic disease patients
  • Identify, manage, and support patients at risk of developing long-term conditions
  • Provide routine nursing care to patients as required, in accordance with clinical evidence-based practice
Requirements:

The ideal candidate will have a Level 7 qualification and prescriber status, with experience in health promotion, chronic disease management, and health prevention. They will also have excellent communication and interpersonal skills, with the ability to work autonomously and as part of a team.
