Job Summary

We are looking for a highly skilled and motivated Advanced Clinical Practitioner to join our team. As a key member of our primary care team, you will be responsible for assessing, diagnosing, treating, and referring patients with undifferentiated or undiagnosed conditions relating to minor illnesses. You will work closely with our GPs, Paramedic Practitioners, Advanced Nurse Practitioner, Practice Nurses, and other healthcare professionals to provide comprehensive patient care.

Main Responsibilities

  • Assess, diagnose, treat, and refer patients with minor illnesses
  • Undertake home visits for acute conditions, as assessed by your duty doctor
  • Use advanced clinical skills to provide education to patients, promoting self-care and empowering them to make informed choices about their treatment
  • Collaborate with our team to provide comprehensive patient care
  • Participate in the administrative and professional responsibilities of the Practice team

Requirements

  • Qualified Advanced Nurse/Clinical Practitioner
  • Experience of working in an urgent care/emergency setting
  • Experience of working in primary care setting
  • Ability to work within a team to provide the best outcome for the patient
  • Flexible attitude to work
  • Clean driving licence
