Hospital Liaison Specialist

We are seeking a dedicated and compassionate Hospital Link Worker to join our team in Stoke on Trent. As a key member of our team, you will play a vital role in providing high-quality recovery care and acting as a liaison between local hospitals and our diverse client group.
About the Role
As a Hospital Link Worker, you will work closely with our clients and local hospitals to establish and maintain a high-quality hospital liaison service for individuals struggling with substance misuse health needs. You will work collaboratively with our Clinical Lead and the hospital's safeguarding team to provide continued support in the community.
Key Responsibilities
- Manage an active caseload of clients, connecting quickly with individuals and treating them with respect and dignity.
- Deliver continuity of care through effective partnership working, care coordination, and delivery of interventions and key work responsibilities.
- Work effectively in a team, supporting colleagues and prioritizing tasks to ensure a high-quality service is maintained.
- Be flexible with working hours on occasion and willing to support other With You locations when needed.
Requirements
- ONC/NVQ level 3 or willingness to achieve within 12 months.
- Experience working in a health, social care, or related setting and partnership working with statutory and non-statutory agencies.
- Knowledge and understanding of health and safety, including assessing high-risk situations for clients.
- Successful track record of working with individuals struggling with substance misuse.
- Ability to build and maintain effective relationships with clients and colleagues.
- Experience completing assessments and risk assessments.
