We are seeking a motivated and caring individual to join our Community Rehabilitation Team as a Rehabilitation Assistant. The successful candidate will work with other health and social care professionals to develop the ethos of multi-disciplinary, integrated care and seamless services, ensuring that patient care is delivered to the highest possible standard.
Key Responsibilities- Implement rehabilitation plans for patients in the community and other settings, observing patient condition and providing direct care with a rehabilitation and therapy focus.
- Support patients with personal hygiene and activities of daily living.
- Work flexibly to meet the needs of the patient group, including working over 7 days and adapting to changing patient needs.
- Contribute to a positive patient experience, promoting independence and well-being.
