About the Role

We are seeking a dedicated and compassionate Support Mentor to join our team at Evolve Housing + Support. As a Support Mentor, you will play a vital role in supporting vulnerable young people in our care, helping them to develop the skills and confidence they need to live independently.

Key Responsibilities
  • Provide one-to-one support to young people, helping them to set and achieve their goals
  • Develop and implement personalized support plans to meet the unique needs of each young person
  • Work closely with other agencies and services to ensure young people receive the support they need
  • Contribute to the development of a safe and supportive living environment
  • Participate in regular team meetings and contribute to the development of the service
Requirements
  • Experience of working with vulnerable young people, preferably in a homelessness or youth support setting
  • Excellent communication and interpersonal skills
  • Ability to work flexibly, including evenings and weekends
  • Knowledge of safeguarding procedures and policies
  • Understanding of the reasons why people become homeless and the needs and challenges they face
