Nurse Case Manager

Childrens Complex Care is seeking a highly skilled and motivated Clinical Case Manager to join our team. As a key member of our community care coordination team, you will be responsible for managing a caseload of children and young people with complex healthcare needs.
Key Responsibilities- Clinical Case Management: Safely manage a caseload of children and young people with complex healthcare needs in the community setting.
- Assessment and Planning: Complete comprehensive assessments and develop bespoke care delivery plans to meet the unique needs of each child or young person.
- Clinical Governance and Risk Management: Ensure high standards of care and service delivery by implementing effective clinical governance and risk management strategies.
- Education and Training: Provide classroom-based training and support to community staff to ensure safe care delivery through our in-house Education Pathway.
- Staff Development and Support: Educate, support, and complete competency assessments for non-registered healthcare staff on clinical delegated tasks.
- Leadership and Accountability: Clinically accountable and responsible for ensuring all reasonable measures are in place to deliver safe, effective, evidence-based care to all children/young people in collaboration with all colleagues.
- UK registered nurse (RSCN or RGN) with strong clinical, management, and communication skills.
- Experience in managing complex healthcare needs in a community setting.
- Ability to work collaboratively with professionals across agencies and sectors.
- Commitment to quality and high standards of care and service delivery.
