Position Overview

As a Senior Nurse Practitioner or Emergency Care Practitioner, the individual will be expected to exhibit advanced clinical skills and possess a comprehensive knowledge base that surpasses traditional nursing roles. The primary focus will be on providing urgent and acute care, both on-site and potentially through home visits, with ample opportunities for professional growth. The role requires working autonomously to deliver high-quality, patient-centered care and to ensure the provision of evidence-based clinical services within the defined scope of practice.

The successful candidate will uphold professional standards, lead by example, and foster a collaborative working relationship with the multi-disciplinary healthcare team, while also assisting management in the evaluation and implementation of clinical policies and procedures.

Main Responsibilities

  • Function as a self-directed practitioner, directing, delivering, and assessing the highest standard of evidence-based clinical care to patients across various settings, including their homes, based on established management care plans and protocols.
  • Operate independently, without direct supervision, conducting thorough patient assessments through history-taking and physical examinations in diverse clinical environments, prioritizing tasks to address the unpredictable nature of this role.
  • Exhibit advanced clinical reasoning and decision-making skills.

About Principal Medical Limited

Principal Medical Limited is a prominent not-for-profit, GP-led organization offering a range of NHS community and primary care clinical services to patients across the region. Since our establishment in 2004, we have experienced significant growth, currently employing around 300 staff and generating an annual turnover of approximately £16 million. We hold GMS contracts and represent a federation of around 50 GP practices, serving a patient population of approximately 650,000.

Key Clinical Responsibilities

  • Collaborate closely with the Clinical Lead to ensure effective service delivery by defining and overseeing service standards in accordance with CQC, NMC/HCPC guidelines, and Principal Medical Limited policies.
  • Conduct home visits and engage with patients, caregivers, and colleagues to evaluate the safety of patients' home environments, taking swift action to mitigate risks.
  • Provide consistent and realistic information to patients and their families.
  • Serve as a clinical resource for patients and caregivers.
  • Work within a multi-disciplinary team to deliver a coordinated, high-quality, and responsive service to patients.
  • Perform consultations as an autonomous practitioner, utilizing clinical judgment to diagnose, treat, refer, and/or discharge patients.
  • Deliver timely, high-quality clinical care.
  • Triage, assess, and diagnose patients effectively.
  • Develop, implement, and evaluate individualized care management plans.
  • For those with prescribing qualifications, prescribe and review medications in accordance with PGD and within the scope of practice.
  • Maintain accurate and comprehensive clinical records for all patient interactions.
  • Make direct referrals to primary, secondary, and social services following established pathways and protocols.
  • Coordinate further investigations as necessary.
  • Collaborate with external services/agencies to ensure appropriate support for patients, particularly vulnerable individuals.
  • Request pathology services as needed, processing and interpreting results as required.
  • Provide supervision and mentorship to other staff to enhance their skills and knowledge.
  • Offer clinical advice and guidance to reception staff as necessary.
  • Understand and adhere to local policies regarding substance abuse and addictive behaviors, making appropriate referrals.
  • Prioritize health issues and intervene as necessary.
  • Recognize, assess, and refer patients with mental health needs.
  • Identify and refer patients with community health needs, developing effective care pathways.
  • Ensure compliance with safeguarding policies at both local and national levels.
  • Provide routine nursing care to patients as required, based on clinical evidence, NICE guidelines, and the NSF.
  • Support the team in managing clinical emergencies.

As a qualified Clinical Practitioner, you will:

  • Adhere to the NMC or HCPC Code of Professional Conduct and practice within the scope of professional practice at all times.
  • Engage in local initiatives aimed at enhancing service delivery and patient care.
  • Participate in shared learning within the organization.
  • Continuously review clinical practices in response to national policies and initiatives.
  • Contribute to the review of significant and near-miss events through structured root cause analysis (RCA).
  • Maintain professional registration and comply with revalidation requirements.
