Job Title: Community Psychiatric Nurse

We are seeking a highly skilled and compassionate Community Psychiatric Nurse to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a Community Psychiatric Nurse, you will play a vital role in providing high-quality care to older adults with mental health needs.

About the Role

As a Community Psychiatric Nurse, you will be responsible for assessing and managing the mental health needs of older adults in their own homes or in community settings. You will work closely with other healthcare professionals to develop and implement individualized care plans that promote the physical and emotional well-being of our service users.

Key Responsibilities
  • Conduct comprehensive assessments of older adults with mental health needs
  • Develop and implement individualized care plans to promote physical and emotional well-being
  • Work closely with other healthcare professionals to coordinate care and support
  • Provide education and support to service users and their families
  • Participate in multidisciplinary team meetings to discuss care plans and share best practices
Requirements

To be successful in this role, you will need to have:

  • Registered Mental Health Nurse (RMN) qualification
  • Minimum of 2 years post-registration experience in mental health nursing
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Knowledge of mental health legislation and policies
