Job Title: Community Psychiatric Nurse

We are seeking a highly skilled and compassionate Community Psychiatric Nurse to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a Community Psychiatric Nurse, you will play a vital role in providing high-quality care to individuals with mental health needs, particularly those with dementia.

About the Role

As a Community Psychiatric Nurse, you will be responsible for conducting initial assessments, managing caseloads, and providing support to clients and their families. You will work closely with our multidisciplinary team to develop and implement treatment plans, ensuring that our clients receive the best possible care.

Key Responsibilities
  • Conducting initial assessments and developing treatment plans
  • Managing a caseload of clients with mental health needs
  • Providing support and guidance to clients and their families
  • Working collaboratively with the multidisciplinary team to achieve high-quality care
  • Participating in team meetings and contributing to the development of care plans
Requirements

To be successful in this role, you will need to have:

  • Relevant formal vocational qualification and first-level nursing registration (RMN)
  • Minimum of 6 months post-registration experience
  • Good IT skills and ability to travel across the locality in a timely manner
  • Experience of working with individuals with mental health needs, particularly those with dementia
