Job Title: Community Mental Health Nurse

We are seeking a highly skilled and compassionate Community Mental Health Nurse to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care to individuals with mental health needs, particularly those with dementia.

About the Role

As a Community Mental Health Nurse, you will be responsible for assessing, planning, and delivering care to individuals with mental health needs. You will work closely with a multidisciplinary team to provide comprehensive care, including cognitive assessments, risk management, and treatment planning. Your expertise will be invaluable in supporting individuals with dementia and their families.

Key Responsibilities
  • Conduct comprehensive assessments of individuals with mental health needs
  • Develop and implement individualized care plans
  • Provide education and support to individuals and their families
  • Collaborate with the multidisciplinary team to ensure seamless care
  • Participate in team meetings and contribute to the development of care pathways
Requirements

To be successful in this role, you will need to have:

  • Relevant formal vocational qualification and registration as a mental health nurse
  • Minimum of 2 years post-registration experience in mental health nursing
  • Experience working with older adults and individuals with dementia
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
