We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, addressing their physical and mental health needs.
Key Responsibilities:- Provide healthcare and clinical interventions to homeless individuals, including assessments, treatment plans, and follow-up care.
- Work collaboratively with outreach teams to identify and engage with individuals in need of healthcare services.
- Develop and maintain relationships with local healthcare providers and services to ensure seamless referrals and care coordination.
- Conduct health education and promotion activities to empower individuals to take control of their health and well-being.
- Participate in quality improvement initiatives to enhance the effectiveness and efficiency of our outreach services.
- Registered Nurse with current registration and a valid NMC PIN.
- Experience working with homeless populations and providing healthcare services in outreach settings.
- Strong communication and interpersonal skills, with the ability to build trust and rapport with individuals from diverse backgrounds.
- Ability to work independently and as part of a team, with a flexible and adaptable approach to meet the needs of our service users.
- Knowledge of local healthcare services and systems, with the ability to navigate and advocate on behalf of our service users.
