About the Role

We are seeking a skilled Care Coordinator to join our team at Sonderwell. As a Care Coordinator, you will play a vital role in ensuring that our clients receive the highest quality of care in the comfort of their own homes.

Key Responsibilities
  • Mobilisation and Resource Allocation: Develop a deep understanding of our clients and carers to ensure effective package filling and resource allocation.
  • Supervision and Monitoring: Conduct regular phone observations and supervisions with carers, addressing any performance issues and ensuring accurate and timely notes on our system.
  • On-Boarding & Interview and Selection: Collaborate with our Talent team to review pipelines, conduct screening interviews, and ensure seamless client-carer placements.
Requirements
  • Experience: Proven experience in a care environment, preferably in a support or coordination capacity.
  • Skills: Excellent communication skills, flexibility, and the ability to self-plan and respond to shifting priorities.
  • Behaviours: A calm and professional demeanor, with a passion for delivering high-quality care and a commitment to our company values.
