About UsAckerman Pierce is seeking a highly skilled Lead Practitioner to join our Social Care Direct Team. This unique, non-caseholding role offers the opportunity to lead and manage a team responsible for handling and directing referrals received via email.
Key Responsibilities- Assess and prioritize incoming emails to Social Care Direct, providing notes and guidance for team action.
- Distribute emails equitably among team members to ensure efficient workload management.
- Offer support and advice to team members on email assignments to facilitate informed decision-making.
- Identify and direct appropriate processes and pathways, including reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge.
- Screen emails meticulously to determine their suitability for referral to Social Care Direct, ensuring compliance with protocols.
- Professional qualifications in Social Work, registered with Social Work England.
- Experience in social care practice, especially in triaging and directing referrals.
- Strong organizational skills for managing email correspondence and workload allocation.
- Excellent communication skills to provide clear guidance and support to the team.
- Knowledge of adult social care processes and pathways, such as reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge.
- Commitment to the council's values of caring, learning to improve, inclusivity, and collaboration.
- Competitive salary between £45,021 and £48,031.
- Hybrid working arrangements, allowing two days of remote work per week.
