We are seeking a highly skilled and dedicated Health Visitor to join our team at Homerton Healthcare NHS Foundation Trust. As a Health Visitor, you will play a vital role in promoting the health and wellbeing of children and families in our community.
Key Responsibilities- Provide health visiting services to diverse children with varied needs, linked to a GP practice.
- Act as the named link to GPs and Early Years Settings & Nurseries, managing vulnerable children and attending multi-disciplinary team meetings.
- Lead on key interventions and health promotion activities, including delivery of integrated development reviews using the ASQ and ELIM questionnaire.
- Oversee the health promotion work undertaken by the nursery nurse team, working closely with parents and children's centres to deliver advice and support.
- Health Visiting professional qualification with entry onto 3rd part of NMC.
- Experience of current developments in Health Visiting and the NHS and Public Health Commissioning.
- Experience of working in a team environment and broad range of clinical experience.
- Knowledge and understanding of the Healthy Child Programme, High impact areas, ASQ Integrated reviews, and current legislation in relation to the profession Child Protection.
- Ability to work flexibly, manage own workload and those of other team members, and engage others and work in partnership across organisational boundaries.
