Our adult social care teams encompass a variety of services, including:
Transition Support Team
This team is responsible for conducting initial adult care assessments and reviews for vulnerable young adults with learning disabilities and their caregivers, collaborating with service users to develop care and support plans tailored to their needs. This role specifically focuses on young individuals transitioning into adult care.
Autism Support Team
The Autism Support Team assesses and aids adults diagnosed with Autistic Spectrum Conditions (ASC), adhering to the Care Act 2014.
Assessment and Reablement Team
This team provides short-term assistance to service users following hospital discharge, closely collaborating with the reablement service as part of the discharge assessment model.
Initial Response Team
This team serves as the primary contact for individuals and professionals seeking assistance from Adult Social Care, whether they are new to the service or previously known but without an assigned worker.
To be considered for this role, you should possess:
- A relevant qualification in social work
- Understanding of the legal framework and procedures governing Adult services
- Knowledge of applicable social work theories and practices
- Proficiency in IT to operate standard electronic systems
- A genuine passion and commitment to supporting the most vulnerable individuals in our community
