Position Overview

We are excited to announce a pivotal role for a qualified Physiotherapist or Occupational Therapist to take on the responsibilities of Clinical Therapy Coordinator within our Early Intervention Team.

In this leadership position, you will play a crucial role in preventing unnecessary hospital admissions and facilitating early discharges from the Emergency Department or Same Day Emergency Care Centre (SDEC). You will collaborate closely with inpatient, community, and primary care partners to enhance patient flow and minimize length of stay for those requiring admission. Your leadership will be instrumental in guiding the Therapy Team, working in conjunction with the Frailty and Discharge Planning Specialist Teams to ensure the delivery of high-quality, evidence-based care.

As a successful candidate, you will be expected to exhibit advanced critical thinking skills in clinical decision-making, identify service gaps, and contribute to service enhancement initiatives aligned with both national and local healthcare strategies. Additionally, you will foster effective teamwork and provide mentorship, education, and support to the Early Intervention Therapy Team.

We seek an individual who is enthusiastic about guiding the team through transitions and advancements. The team operates seven days a week with extended hours.

Key Responsibilities

  • Collaborate with the Service Lead and other senior Clinical Leads to evolve the service in response to the dynamic health and social care landscape through regular audits and data analysis.
  • Assist fellow EIT Clinical Leads in delivering training and support to EIT staff, Frailty Specialist Nurses, and other team members, including Physiotherapists and Occupational Therapists.
  • Provide clinical supervision and promote professional growth within the Therapy team.
  • Encourage a proactive and positive approach to risk assessment in admission avoidance and discharge planning, supporting junior Therapists in this endeavor.

Role Responsibilities

The Early Intervention Team (EIT) is a forward-thinking, multidisciplinary group comprising Frailty Specialist Nurses, Discharge Planning Specialist Nurses, Occupational Therapists, and Physiotherapists. The EIT has gained recognition for its proactive Admission Avoidance service, which includes Frailty screening and comprehensive geriatric assessments for patients in the Emergency Department, Urgent Treatment Centre, and Medical Assessment Units.

Qualifications and Experience

Essential Qualifications

  • Registered Physiotherapist or Occupational Therapist with a degree and a commitment to pursuing post-registration qualifications in the specialty.
  • Evidence of ongoing professional development.

Desirable Qualifications

  • Research Module completion.
  • Master's Degree.

Essential Experience

  • Extensive post-registration experience in an acute hospital environment.
  • Experience in relevant specialties such as Orthopaedics, Medical, Respiratory, Frailty, and Neurology.
  • Competency in a transdisciplinary work setting.
  • Proven experience in multidisciplinary teamwork and leadership.
  • Ability to thrive in high-pressure, fast-paced environments.
  • Experience in supervising and appraising staff and students.
  • Involvement in audit processes.
  • Participation in quality improvement initiatives and change management.
  • Experience in formal and informal teaching.

Essential Knowledge

  • Understanding of evidence-based practice and clinical governance.
  • Knowledge of patient pathways and complexities in supporting patients.
  • Familiarity with current best practices in therapy within emergency care settings.
  • Knowledge of best practices in therapy for Frailty, Medicine, Neurology, Orthopaedics, and Pain Management.
  • Awareness of legal responsibilities and ethical codes of the profession.
